Article 16 Referral
For Article 16 Clinic Services, including counseling, maintenance therapies, psychological and psychosocial evaluations, and sexuality assessments, a referral from a family member, services coordinator or care provider is required. You may complete and this form electronically by printing it here (Article 16 Referral Form) and emailing it to referral@heartshare.org (referral@heartshare.org).

For those who cannot travel by public transportation, door-to-door transportation can be arranged.
