Insurance Information
Most HeartShare Wellness patients have Medicaid and/or Medicare insurance. In addition, some individuals may have private insurance. Please call the clinic for insurance verification and acceptance. For uninsured patients a self-pay account can be set-up.
All insurance cards must be brought with the patient at the time of the visit.

Same day appointments are available. However, you must contact us to schedule an appointment.

For those who cannot travel by public transportation, door-to-door transportation can be arranged.
