Overview
Since opening in May 2000, HeartShare Wellness has provided high quality health care to thousands of individuals with intellectual and developmental disabilities. Located in a beautiful, state-of-the-art facility in Downtown Brooklyn, HeartShare Wellness is a diagnostic and treatment center that offers its patients medical, specialty, rehabilitative, counseling, and evaluation services in one convenient location.
Too often, individuals with disabilities and their families and caregivers have had to make time-consuming multiple trips to various locations to obtain needed health care. HeartShare Wellness offers coordinated care where highly trained physicians and clinicians value our patients' time and recognize their unique needs.
Our physicians and clinicians also keep the lines of communication open with the family, school and other care providers to ensure the patient's overall well-being.
No matter what the level of cognitive or physical disability, and no matter whether the individual is able to indicate their needs independently or requires assistance, the HeartShare Wellness staff provides the care and attention that fits the individual's total needs.
In addition, HeartShare Wellness is pleased to offer services that may not be available with other medical or clinical providers, including pediatric psychiatry, gynecology, optometry, neurology and podiatry. We are also well-known providers of individual and group counseling, psychological and psychosocial evaluations and sexuality assessments.

Convenient to B26, B38, B41, B52 and B54 busses, as well as 2, 3, 4, 5, A, E, F, N and R subway lines.
For those who cannot travel by public transportation, door-to-door transportation can be arranged.

- Monday, Wednesday and Thursday: 9am - 8pm
- Tuesday and Friday: 9am - 5pm
- Saturday: 10am - 5pm
